We appreciate the authors of this article titled A discordance aware multimodal framework with multi-agent clinical reasoning for putting this scientific work together. We have gone through this scientific piece and have put together the feedback below.

Major concerns

The aims and objectives of the study are not clearly stated. As this is a scientific study, the authors should explicitly define the primary aim and specific objectives to clarify the purpose of the research and ensure that the methods, analyses, and conclusions are appropriately aligned with the intended objectives.

The sample size may be insufficient to support external validation and generalizability of the findings. Clarify how the sample size was determined, including whether the analysis was based on the number of individuals or the number of knees assessed. Although most individuals have two knees, some may have undergone amputation, meaning that the number of knees assessed may differ from twice the number of participants. Report the total number of individuals and knees included in the analysis, as applicable, and justify whether the sample size is adequate for external validation. If the sample is relatively small, the authors should acknowledge this limitation and address claims regarding the generalizability of the findings.

The sampling approach may introduce selection bias, as knee osteoarthritis prevalence and progression vary by age, sex, menopausal status, obesity, and other relevant risk factors. The manuscript does not clearly describe the distribution of these characteristics in the FNIH Osteoarthritis Consortium dataset or explain how they were accounted for in the analysis. Provide a more detailed description of the study population, including relevant demographic and clinical characteristics, and clarify whether potential confounders were considered in the analysis. If these variables were unavailable or could not be adequately addressed, acknowledge this limitation and discuss its potential implications for the interpretation and generalizability of the findings.

The manuscript reports the area under the curve for the prediction models but does not provide corresponding confidence intervals. Confidence intervals are important for assessing the precision and uncertainty of model performance estimates and determining the reliability of the reported findings. Report the 95% confidence intervals for the area under the curve estimates and for other key performance measures. The authors should also explain how these estimates were obtained. Reporting confidence intervals would help readers assess the precision of the prediction models and better judge the extent to which the findings may be generalizable to other populations.

Minor concerns

The limitations of the study are not adequately discussed. Please include a dedicated section identifying the key methodological and analytical limitations, such as potential sources of bias, sample size constraints, data availability, and limitations affecting the interpretation or generalizability of the findings, where applicable. The authors should also explain how these limitations may influence the results and conclusions. Transparent reporting of limitations will help readers assess the strength of the evidence and inform the design of future studies.
