PREreview of Improving Antenatal Syphilis Screening and Treatment Coverage in Public Health Facilities in Kampala, Uganda

1. Summary

This study examined whether an intervention could improve syphilis screening and treatment among pregnant women in Uganda. The researchers compared data from before and during the intervention.

The results showed that syphilis screening increased from 55.2% to 88.3%, and same-day treatment among women who tested positive increased from 74% to 100%. Partner testing also improved, but there were still challenges in providing treatment for infants with confirmed or possible congenital syphilis.

2. Strengths

One strength of this study is that the researchers collected data from public health facilities. I think this is important because the results show what happened when the intervention was used in real healthcare settings. The study also included a large number of pregnant women, which makes the findings more useful.

Another strength is that the researchers did not only focus on pregnant women. They also looked at partner testing and infant treatment. I like this because preventing congenital syphilis is not just about screening pregnant women. Making sure their partners and infants receive care is also important.

3. Major Comments

1. Lack of a control group

My main concern is that the study did not have a control group. Although screening and treatment rates increased, I wonder whether other factors could have affected the results. For example, healthcare services or staffing might have changed during the study period.

I think it would be helpful if the authors explained whether there were any other changes in the healthcare facilities during this time. Without a control group, it is difficult to know whether the improvements were mainly caused by the intervention.

2. Same-day treatment and treatment completion

Another thing I noticed is that receiving treatment on the same day does not always mean that patients completed their treatment.

I wonder whether the researchers had information about treatment completion. If they did, I think it would be helpful to include this information or explain it more clearly. This is important because receiving treatment and completing treatment are not always the same.

3. Multiple intervention strategies

The intervention included several strategies, such as rapid testing, treatment, and partner notification. Since these strategies were introduced together, it is difficult to know which one was the most helpful.

I think it would be useful if the authors discussed this limitation. It might also be helpful to explain which strategies could be easier for healthcare facilities with limited resources to use.

4. Do the Results Support the Authors' Conclusions?

I think the results generally support the authors' conclusions because screening and treatment rates increased during the intervention. However, I think the authors should be careful when saying that the intervention caused these improvements, since other factors might also have contributed.

I think explaining this limitation more clearly would help readers better understand the findings.

5. Suggestions for Future Studies

For future studies, I think it would be helpful to include a control group to better understand the effects of the intervention.

I also think it would be interesting to explore which intervention strategies were the most helpful, especially for healthcare facilities with limited resources.

6. Overall Impression

Overall, I think this is an important study because syphilis during pregnancy can affect both mothers and infants. I also like that the researchers considered different parts of healthcare, including screening, treatment, and follow-up.

Although the study has some limitations, I think the findings are useful for understanding how
