An applied empirical extension of Human Rights+ (HR+). This reflexive qualitative case study uses a documentary case file to show how Harm Attribution Gaps and institutional processes can produce cumulative attrition and invisible displacement without formal expulsion. It traces procedural escalation, category constraint, epistemic invalidation, and accountability diffusion, including post exit non engagement with documented prevention relevant information. The paper contributes an evidence grounded mechanism map to the HR+ series and advances Perceptual Justice as a recognition layer for consequential but non actionable harm.
