Introduction: Back pain affects functionality, quality of life, compromising the performance of occupations. The first care approach is assessment, which is complex, making the use of assessment tools essential for therapeutic reasoning and implementation of the practice. There are still few assessment tools in occupational therapy for spinal conditions. Objective: To validate an instrument for assessing and diagnosing the impact of the occupational performance column. Method: Analytical, descriptive, cross-sectional study with a quantitative approach. Describes the systematization, content validation, Delphi technique, and semantics of an instrument to assess the impact of the spine on occupational performance. Content validation was carried out virtually, in three stages, with a jury of occupational therapists who are specialists and/or working in the physical area. The semantic validation was performed with the association of spine carriers, through a closed questionnaire. Results: The agreement index described as a validation criterion was achieved. In the semantic validation obtained 100% positive overall impression. Final Considerations: This study strove to build an instrument for the use and facilitation of occupational therapy, the adoption of parameters that guide the conduct and reliability of care and increase the quality of services provided.
